Asthma/Anaphylaxis Action Plan Required

Welcome Back! We have many students at Missouri Valley Schools with asthma or anaphylaxis (a severe allergic reaction). If your son or daughter is currently using an inhaler or requires an epi-pen, we will need an Asthma/ Anaphylaxis Action Plan form completed by their medical provider. We would greatly appreciate your cooperation in having this form completed and returning it to school as soon as possible. An emergency care plan will be developed for those students who have an Asthma Action Plan directive on file. A student’s health and safety is our top priority. Feel free to contact one of the district nurses with any concerns or questions.
